Referral Forms

There are three main forms:

• Referral Form for other mental health professionals and agencies
• Equine Liability Form for parents and others
• Medical Information Consent Form

If you are a mental health provider referring a client, you need only fill out the "Referral Form." All clients or their parents will need to fill out the "Equine Liability Form" and the "Medical Information Form," both of which can be completed at the initial visit.
